Overview:

Over 5 million Americans are completely or mostly homebound or have significant limitations that make it extremely difficult to leave their home. Access to appropriate care and services can be extremely difficult – many times compounding the complexities of their cases. These patients, many times unseen by the “traditional” clinical setting, can fall through the cracks, leaving their loved ones, friends, and caregivers with the insurmountable task of caring for patients that require an interdisciplinary team of specialized care. The award-winning documentary film, The Invisible Patients, tells the story of Jessica Macleod, a nurse practitioner, and four homebound patients she cares for in Evansville, Indiana. Together they put a human face on some of the most urgent health care issues facing our nation, from the living conditions of the elderly poor and end-of-life care to the soaring costs of hospitalization, the complexity of insurance and over prescription of opiates. To supplement the eye-opening information from the film, we will also delve into gaps in the continuum of care, barriers to quality care in the home, best practice models, and considerations and guidance for providers working in healthcare. Clinicians will have a better understanding of the overlapping barriers and challenges that many homebound patients face, and gain the valuable tools necessary to enhance practice, advocate for programs and services that meet needs, and connect patients/caregivers to those resources. Together, providers can shine a light on the most vulnerable in our communities and ensure that their health and wellness does not go unseen.

Patrick O’Connor, a writer, producer and director who primarily works in marketing for healthcare systems, as well as numerous non-profit organizations, through his company o’connor/creative. His feature film writing credits include Ricochet River, produced by Gigi Pritzker and starring Kate Hudson, and the independent feature Sacred Hearts, which he also directed.  Sacred Hearts premiered at the Boston Film Festival and subsequently played at the Fort Lauderdale Film Festival (awarded Best First Feature) and the Lincoln Center in New York. His screenplay, ZOO, was purchased by Steven Spielberg’s Amblin Entertainment.

It’s no secret that healthcare costs in the U.S. are skyrocketing. Communities and healthcare systems have had to look at ways to reduce expenditures and yet continue to improve patient care. With the passage of the Affordable Care Act, hospitals are financially penalized if they have excessive readmissions rates. With this increased awareness of the prevalence of hospital readmissions coupled with the new financial penalties, health systems have employed intense and widespread efforts to reduce early hospital readmissions. Research suggests that the particular strategies chosen should be based on quality data, and with the oversight of a multidisciplinary team within a well-connected care delivery system. Furthermore, all strategies must be coupled with a system-wide commitment to supporting the patient’s capacity for self-care.

Overview:

Veterans can often feel like “unsung” heroes. For many, after they’ve served our country they return home only to experience a gap in resources and opportunities. As healthcare professionals, we may set out to help the veterans we encounter in our daily work, but soon we too face scarcity in resources for our veteran clients. This online course will highlight the challenges our veterans face with a personal experience speaker who share his experience as a United States Air Force Veteran. Tyler Bowser, Veteran Community Ambassador with the Veterans Empowerment Organization (VEO), will set the tone with an inspiring story of trial and triumph. The course will also provide an overview of the U.S. veteran population, and the service and benefit systems available to them.  The importance of coordinated, trauma-informed care, as well at the opportunities and challenges facing providers will be outlined.  Finally, participants will receive a comprehensive list of Federal and local resources to reference in their work with our veteran heroes.  Participants will leave inspired, empowered, and better equipped to help our veterans!

Overview

Why is self-care so important? And who could imagine that you could be unethical for not practicing self-care? As helping professionals, we inform our clients/patients of the importance of self-care, while many of us are overworked and under-performing due to compassion fatigue or burnout. Through a TED Talk about The Power of Vulnerability by social worker, storyteller, and researcher, Brene Brown, and a conversation about self-care by Anne McSweeney, LMSW and Jenny Buckley, RN — participants will learn self-care strategies, ethical codes pertaining to self-care and the importance of self-care as a prerequisite for adequate patient/client care. Participants will leave with a take-away reminder that will keep you encouraged to care for yourself all throughout the year!

In this presentation, we’ll get oriented to essential challenges, terminology, and the legal-ethical-professional environment of telemental health. We’ll focus on making sure that attendees know what they don’t know and have the resources to fill in the gaps. Confronted with a U.S. population that is growing, aging and facing deteriorating health, the eye-opening documentary, The American Nurse, showcases 5 fearless nurses as they serve on the frontlines of healthcare — navigating their patients through birth, death, prison, war and harsh poverty.  In addition, with the COVID-19 Pandemic in 2020 – the front lines of healthcare (many of whom are nurses) are strained emotionally – suffering from severe psychological trauma.  As clinicians and helping professionals – it is imperative that we have a deeper understanding of what they go through “on the front lines” as we provide them with counseling, social work and other services.   From award-winning filmmaker Carolyn Jones, the film will leave you informed and inspired by the amazing nurses that serve our communities every day.  Supplemented by an overview of the profession — with demographics, historical considerations, and challenges and opportunities, this online course will provide participants with a better understanding of the nursing profession, its vital role in our healthcare system, and the critical linkage nurses play in building and sustaining healthy communities.  In viewing this eye-opening documentary film, the clinician/helping professional will gain better insights into what healthcare professionals go through on a day-to-day basis and thus, gain insights into being able to provide better treatment and resources to these clients in need.

• Identify at least two resources to help counselors, social workers and other helping professionals when dealing with a nurse (or front-line worker) as a client.

Carolyn Jones

Carolyn Jones, an award-winning photographer and filmmaker who specializes in telling stories that shed light on issues of global concern. Her first book, Living Proof: Courage in the Face of AIDS, was published by Abbeville Press and accompanied by shows in Tokyo, Berlin, the USA, and at the United Nations World AIDS Conference. She directed a television series for Oxygen Media called Womenshands as well as Women… on Family, a program for PBS. Carolyn founded the non-profit 100 People Foundation which creates educational films and curricula for school children worldwide. She has spent the last two years interviewing nurses from all over the country for the book and documentary film: The American Nurse.
